Transformations Care
Our Addiction Treatment Process
It All Begins with a Phone Call
We can help, but the first move is yours to make. Connect with us and let us know what you or the person you love needs help with. We’ll tell you what we do and how we can help you or the person you loved with addiction
Evaluation for Addiction Treatment Process
Our careful evaluation process begins with a clinical screening interview over the phone. This call must be directly with the person who is seeking addiction treatment themselves so we can get their personal experience.
During the phone interview we just want you to give it to us straight. Don’t hold anything back or leave anything out. You won’t shock or disappoint us with anything you say.
Remember that the more relevant info you can give us, the better. We are a dual-diagnosis program. That means we treat co-occurring disorders like depression, anxiety, OCD, BPD and so on.
Be sure to let us know about any diagnosis you’ve had from a mental health professional along with your substance use history. We’ll also want to hear about previous treatment experiences and any relevant medical history. This is a no judgement zone.
That’s only the very beginning of the evaluation process though. You will be continuously evaluated along the way in different ways so we can both chart your progress and make any necessary adjustments to treatment.
Just some of the evaluation tools and methods we use include:
- The COWS chart (in detox)
- Medical Physical Evaluation
- BioPsychoSocial Evaluation
- Psychiatric Assessment (if applicable)
- Therapists Notes and Charting
- Discarge and Aftercare Plan Recommendations.
Preparing for Treatment – Addiction Treatment Process
Once your intake is approved, we will schedule your admission. Be sure to stay in touch with our admissions department throughout that time. Feel free to ask any questions about what to bring (or not bring).
You will find some helpful notes on our Admissions Page. Packing a bag for addiction treatment process is easier than you probably think it is. You can us a call when you’re about to pack to go over some stuff of course, and we recommend that. In the meantime, though, here’s some tips to get you started.
DO BRING:
- Health insurance card and one form of government issued photo ID.
- A phone and/or list of important contact phone numbers for family etc.
- Any non-narcotic prescription meds you take in the original bottles.
- 7-10 days’ worth of comfortable, casual clothes (T-shirts, shorts, sweats, hoodies)
- Comfortable sneakers/walking shoes
- Extra socks and underwear (seriously, when is this ever not a good idea, right?)

DON’T BRING:
- Revealing clothing, booty shorts, thong underwear or bathing suits, crop tops etc.
- Clothes that promote drugs or alcohol, like beer T-shirts etc.

The Patient Experience at Transformations Care from the Top | Addiction Treatment Process
Part 1: Detox
Most patients who are actively using are going to being in the detox phase of care. We strongly suggest not attempting this at home. Trying to quit cold turkey is crazy, not brave. It also doesn’t help your chances of staying sober. It actually makes them worse, in fact. If you’ve failed to muscle through a cold turkey detox that doesn’t make you weak, it makes you human.
Look, you don’t deserve to suffer. Not even if you’ve made some bad decisions. Suffering needlessly is pointless. It’s not price of recovery either. If you are here and looking for addiction help, you’ve already suffered enough.
Transformations Care has a peaceful and comfortable medication-assisted detox with just 6 beds tucked away in a quiet cul-de-sac in the Los Angeles suburb of Gardena. We’re in recovery ourselves so we know how important a safe and comfortable detox is. We’ve got you.

The bulk of your recovery work with us begins in our residential phase of addiction treatment process. It is where you’ll experience breakthroughs with your therapists and learn a whole new perspective.
We have a no-nonsense program with a hearty blend of holistic and traditional addiction treatment process. Our comfortable accommodations foster healing and peace of mind. We practice evidence-based addiction care here and always have. In plain terms, that means we do stuff that research proves helps people get sober and stay that way. Period.

Part 3: Partial Hospitalization Program (PHP)
The Partial Hospitalization Program is the level of care that is one step down in intensity from Residential Inpatient Treatment. It usually features many of the same treatment methods, like individual and group therapy and other activities.
The main difference is that clients no longer stay in a medical facility overnight. Most clients in a Partial Hospitalization Program will either live at home if they are local. in a nutshell, our Partial Hospitalization Program includes many or most of the features of our residential addiction treatment process, except there are a bit fewer hours of treatment per week overall (but still a full week’s work) and patients do not sleep in a clinical facility with 24-hour medical staff (but there are 24-hour non-clinical staff in the supportive sober houses we refer to)
Transformations Care Partial Hospitalization Program in Los Angeles features:
- A full day of drug and alcohol treatment.
- Holistic addiction care including art therapy and wellness.
- Proven 12-step fellowship recovery support components (AA & NA)
Part 4: Intensive Outpatient Program (Iop)
Our job is to get you healthy, help you understand yourself better and give you tools so you can keep growing and getting stronger. Our IOP helps you start to practice new recovery skills while living in supportive sober housing if you choose.
We even offer evening IOP treatment for people who need more flexible scheduling. IOP is an evidence-based addiction treatment process that is proven to improve outcomes for people with substance use disorders.
Transformations Care Intensive Outpatient Program in Los Angeles features:
- Evidence-based Intensive Outpatient drug and alcohol treatment.
